This form issues a credential to a named person at an organisation. It is also the qualification step: the fields below the fold replace the call that would otherwise have to happen before anything useful could be sent to you.

Read this first: 108 of the 491 documents in the register need no credential at all, and a further 128 open once you accept the mutual NDA and an administrator approves your request. Most security reviews finish without ever using this page.

Your hospital

Every field here is optional. They exist so that the qualification call does not have to happen at all. Answered now, the first thing you receive is a plan with your dates on it rather than a request for a meeting.

Licensed or operational, either is fine.

Sets the stamp duty position and the governing courts.

This predicts the signature path better than anything else on the form, and the signature path sets the schedule.

The product name, or the word none. It decides whether a migration sits on the critical path.

A date you would like, not a commitment you are making.

The person who signs a contract for the hospital. A name and a designation is enough.

Optional and rarely known offhand. Your hospital holds its own HFR, HPR and NHCX credentials; Pensieve integrates under them rather than in its own name.

Who else will read the paperwork?

Naming them now means their copies go out with yours instead of a week later. External advisers do not consume one of the five workspace seats.

What happens to this

Submitting this form records your details for one purpose: to decide your credential, and to contact you about the documents you asked for. It will not be sold, will not be enriched from third-party sources, and will not be used to profile your organisation. The decision, the reason and the time it took are written to an append-only audit log. You can ask for all of it to be erased at any time, and the erasure is self-serve. See your data.
